feat: support MII and MIRACUM entlassgrund fix: #54
Add entlassgrund information to encounter using a dual coding (MII & MIRACUM), also use both ways (extension and dischargeDisposition). FHIR example way has not been changed and is still added. The entlassgrund code is splitted, the second part is added as an extension (MIRACUM only as not supported by MII). Closes #54 (closed)
